Please join us to learn about Age-Friendly Health Systems and the 4Ms of Age-Friendly Care, an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement (IHI). This program will include presentations from IHI on state-based efforts to advance age-friendly care, as well as a case study discussion from a Massachusetts healthcare organization that has implemented the 4Ms Framework with the goal that all care with older adults is age-friendly.
